Tufts CSDD has reported average Phase III budgets above $65 million for oncology protocols and $54 million for non-oncology protocols (Tufts CSDD Impact Report, 2022). A 2022 Tufts CSDD survey found that 76% of protocols now have at least one amendment (Tech Funding News, citing Tufts CSDD), and 83% of sites prefer to be paid within 30 days (Clinical Leader).
Budgets that large, that change that often, paid to sites that need cash quickly, cannot be managed from spreadsheets updated once a month. Sponsors need four things from clinical trial financial management (CTFM) software: a real-time view of spend, a forecast that follows the study, site payments that are right first time, and control over change orders. Here is what each looks like in practice.

|
Where will we land? |
A forecast built from actual enrolment, visits and milestones |
|
Are we paying sites correctly? |
Payments calculated from completed visits and contracted budgets |
|
Do we need a change order? |
A forecast that tests the impact before the change order is agreed |
Spend is only visible in real time if the data that creates it is in the same system. When completed visits and milestones generate earned amounts automatically, finance and clinical operations can see earned, invoiced and paid amounts at any point, broken down by region, country, site and vendor, without waiting for month-end reports.
A forecast built on the original plan drifts as soon as enrolment differs by site. Forecasting from actual enrolment, completed visits and study milestones, such as site activation, first patient in or last patient out, shows where spend will land and when. That is also what accruals and cash planning depend on.
Accurate payments start with the right data: the visit that was done, the contracted fee for that site, and the payment terms, holdbacks and overhead in its contract. Paying from completed visit data removes most of the errors that cause disputes and delayed invoices. Pass-through costs on site invoices still need checking, line by line, against the budget.
With 76% of protocols amended, change orders are part of almost every study. A forecast that can model the impact of a change, by region and site, shows whether a change order is needed and roughly how much before the vendor's quote arrives.
Cloudbyz Clinical Trial Financial Management (CTFM) is built natively on Salesforce on the same platform as Cloudbyz CTMS and EDC, so visits, milestones, enrolment, budgets and payments share one data model. Completed subject visits generate invoices against site budgets, amendments are applied to the affected visits and procedures, and sponsors and sites see status through portals.
The CTFM forecasting module uses actual study progress to forecast spend, and can model scenarios to show whether a change order is needed, how much, and which regions and sites are affected, then track spend against it. On the same platform, the Financial Navigator, an Agentforce agent, answers questions on payments, invoices and budgets and flags invoices ageing past 90 days, read-only. The Invoice Reconciliation Agent is designed to check site invoice lines against the proforma and route exceptions to a person.
