And then we hand a 200-page protocol to a site coordinator and say: execute this perfectly, from memory, for every subject, at every visit, for two years.
They can't. Not because they're not capable. Because the system was never designed to help them.
What we've built over the last two decades is an infrastructure for recording what happened. CTMS captures visits. EDC captures data. eTMF stores documents. Every system in the eClinical stack is optimized for documentation and almost none of it is optimized for execution. CRAs discover problems after the fact. The monitoring visit is designed to catch what went wrong out-of-window visits, missed procedures, undocumented deviations. A CRA monitoring 6 to 8 sites spends 2 to 3 hours preparing for each site visit, manually cross-referencing visit dates against protocol windows, checking whether PD records exist for known issues, and building a picture of site performance from scattered data. By the time the CRA finds the problem, the deviation has already happened. The only question is how well it gets documented.
Project Managers lack real-time visibility. Study level compliance is typically assessed in monthly or quarterly governance meetings, assembled from reports that are already outdated by the time they're presented. A PM managing a multi-site study has no way to see, today, which sites have subjects approaching visit window edges, which visits are generating the most deviations, or where the next compliance risk is likely to emerge. They manage by looking backward.
Sponsors get the picture last. Oversight reports arrive aggregated, summarized, and delayed. These aren't data entry errors. They're execution errors that manifest as data quality issues. Cleaning the data doesn't fix the process that created the problem.

A site coordinator needs to confirm what procedures are required at Visit 7. Instead of opening the protocol PDF, finding the SOA table, and interpreting the abbreviations, they ask the system: "What procedures are required at Visit 7?" The system responds with the specific procedures, sourced directly from the protocol SOA, with the page reference cited.
A CRA wants to understand the visit window definition for the end-of-treatment visit. The system retrieves the relevant protocol section the target day, the allowed window, any special conditions and presents it with the source reference. This protocol intelligence layer is available anytime not just when someone is sitting at a workstation, but as a reference that can be accessed through the platform wherever the user is working. The protocol stops being a document you have to search and becomes a knowledge base you can query.
None of this is conceptually revolutionary. The individual capabilities visit tracking, window monitoring, deviation detection, checklist generation, protocol search are logical extensions of what CTMS platforms should have always done.
What's different is the orientation. Every capability in this model is designed around prevention, not documentation. The system's primary job is not to record what went wrong. It's to prevent things from going wrong in the first place. And when they do go wrong, it ensures they're caught and documented immediately not weeks later, not during monitoring, not during an inspection. This is quality by design applied to trial execution.
